To create a survey-like form, you can utilize signup forms and custom form fields. Signup forms allow you to collect information from users, such as their name and email address, while custom form fields enable you to gather specific data related to your survey.

Before you get started

Make sure you have access to the messaging menu and Custom fields
Please note that custom fields are not available on RSVP pages

How to create a survey using a Signup form

1. Create Custom Form fields to add to your form

Form fields are ideal for collecting responses to questions like a survey, such as 'what actor do you want to see most?'

2. After creating the Custom Form Fields, go to the messaging menu, click on the Signup Form tab

3. Click create new signup form

4. Go to Step 2: Form Fields

5. Scroll down and click +Add Custom Field

6. Enter the Field Name and Field Type

7. Click Save
