To create a form that resembles a survey, consider creating a signup form that includes Custom Fields.
Before you get started
- Make sure you have access to the Messaging Menu and Signup forms.
- Please note that custom fields are not available on RSVP pages
How to add Custom Fields in your signup form
- Create a Signup form, or click Edit Signup form to open a current form
- Go to Step 2: Form Fields
- Scroll down and click Add Custom Field
- Enter the Field Name and Field Type
- Scroll to "Dropdown options" and apply tags to each option. These tags will automatically be applied to contacts who select this option.
- Click Save
*For best practice to enable segmentation, we recommend applying tags to each of those custom fields*
How to remove a Custom Field in your signup form
- Click Edit Signup form to open your current form
- Go to Step 2: Form Fields
- Scroll down to the Custom Field
- Switch the Toggle to OFF to remove it from your signup form page
How to add existing custom fields to your Signup form

- On the second page, scroll to +Add custom field
- You can select your existing custom field(s)
- Click Save
Populating Custom Fields
- Via a Signup Form: As a contact completes this, their responses will be populated accordingly.
**Please Note: There will be a small delay between the import completion and the custom fields being visible. This is expected and should not be cause for alarm.
